BeWell 2017-2018 Scorecard Overview The HCESC BeWell Scorecard is our way of helping all HCESC employees be aware of and take charge of your personal health. We want to help you achieve your health goals and reward you for doing so! For the 2017-2018 scorecard, the BeWell incentive dollar amount you can earn is $200.00. We have multiple activities you can choose in order to earn your BeWell Incentive. For the 2017-2018 BeWell Scorecard, we have again partnered with The Christ Hospital. Through their new online “Applied Health” platform you will have: secure, confidential access to your health information through their Personal Health Portal the ability to track your scorecard items access to suggestions for improving your health the option to earn additional incentive dollars If you choose to participate and want to earn your BeWell incentive, you MUST complete these three required components. If you complete all three you are eligible for the BeWell incentive minimum award of $120.00. BeWell Scorecard required items: This must be completed FIRST: Personal Health Survey online (will take approximately 15 min to complete) Complete your biometric measurements by either: Attending a Biometric Event (at one of the HCESC locations) OR Visiting your physician, completing your biometric measurements, and submitting verification (form found on portal) Have an annual physical or wellness visit with healthcare provider and submit verification of the appointment (form found on health portal) Once you have completed the first three components, you will be eligible to earn up to an additional $80.00 (maximum) by participating in a wide range of optional components. You can begin completing optional items before/while completing required items.
